Job summary

Qualifications

• Master’s Degree in Social Work (MSW) or related field from an accredited university required.
• Licensed Clinical Social Worker (LCSW) or equivalent licensure preferred.
• Case Manager Certification (CCM) preferred.
• 3 or more years of experience with Medi-Cal population, medical social work, community clinics, or similar.
• Strong analytical and problem-solving skills.
• Experience conducting psychosocial assessments and care planning for complex clients.
• Proficient in Microsoft Office (Excel, Outlook, Word, PowerPoint).
• Excellent communication skills, both verbal and written.
• Knowledge of health disparities in diverse communities.
• Effective time management and prioritization.

Responsibilities

• Complete member assessments related to social drivers of health and access to healthcare needs.
• Collaborate with providers to support treatment plans and care needs.
• Respect member autonomy and informed decision-making in care planning.
• Assess health behaviors and cultural influences to identify barriers to treatment.
• Manage a caseload of high-severity members and provide supports and linkages.
• Coordinate services with state/community programs when needed.
• Document psychosocial assessments and referrals timely.
• Participate in Interdisciplinary Team Meetings and case consultations.
