Blue Shield of CA is seeking a Social Service Representative, Advanced to provide telephonic social services support and resource referrals for members, while assisting the Social Services Management Team. The role involves outreach to members and families to assess needs and coordinate care improvements.

Candidates should have 5 years of relevant experience, Medi-Cal/Cal MediConnect knowledge, and strong communication skills. Hybrid virtual work is available, with occasional in-office needs.

Qualifications

• High School diploma or GED or 1 year of additional relevant experience in lieu of a diploma.
• Requires 5 years of relevant experience
• Demonstrated 2 years of experience with Medi-Cal managed care including Cal MediConnect and Medicare
• Bachelor's Degree in Social Work or equivalent preferred
• Proficient skills with computer programs, including Microsoft Excel, Outlook, Word, and PowerPoint
• Excellent communication skills both orally and in writing
• Knowledge of health disparities in diverse communities
• Effective time management skills and ability to define and act on priorities amid multiple responsibilities
• Experience working with Medi-Cal populations preferred
• Bilingual Spanish speaking preferred

Responsabilités

• Provide telephonic social services support and resource referrals for Blue Shield Promise members.
• Coordinate with team members and other internal departments, and provide first line of telephonic response for Social Services.”
