Job summary

Blue Shield of CA is seeking a Senior Social Worker to assess social drivers of health for Promise Health Plan members and coordinate intensive interventions. You will connect members with community resources, support care management, and navigate LTSS and public programs.

The role requires MSW and LCSW licensure or equivalent, with 3+ years in Medi-Cal or similar settings, strong analytics, and bilingual Spanish preferred. Hybrid virtual work is available.

Qualifications

• MSW from an accredited university is required.
• LCSW or equivalent licensure preferred.
• CCM certification preferred.
• 3+ years of experience with Medi-Cal population or related settings.
• Strong analytical and problem-solving skills.
• Experience conducting psychosocial assessments and care plans for complex clients.
• Knowledge of LTSS and related programs and eligibility.
• Proficient in Excel, Outlook, Word and PowerPoint.
• Excellent oral and written communication skills.
• Knowledge of health disparities in diverse communities.
• Strong time management and prioritization abilities.
• Bilingual Spanish speaking preferred.

Responsibilities

• Outreach to Blue Shield Promise Health Plan members.
• Conduct assessments for social drivers of health needs.
• Provide intensive interventions and care planning.
• Linkage to community resources to reduce barriers to care.
• Support members to manage medical care and community living.
• Apply knowledge of LTSS and public programs in care planning.
