Job summary

West Valley Post Acute is seeking a Staff Speech-Language Pathologist to evaluate residents, develop and update individualized treatment plans, and collaborate with physicians, nursing staff, and families to support resident goals and discharge planning.

The role includes supervising CFY or student clinicians, delivering staff in-services, timely documentation, infection control, and educating residents and families on home exercise programs.

Qualifications

• Bachelor's degree in Speech-Language Pathology; preferably Masters or Doctorate in the field.
• Proficient with Net Health and Point Click Care.
• Licensed Speech-Language Pathologist in the state; maintain licensure and CPR certification if required.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
• Assist nursing with training of Restorative Aides and supervise CFY or speech students.
• Communicate with physicians, nursing staff, and families regarding progress and discharge planning.
• Document timely, accurate, and compliant records in accordance with facility and payer requirements.
• Educate residents, families, and caregivers on goals, home exercise programs, and safety techniques.
