(Waverly Hills Post Acute) Waverly SNF Healthcare, LLC is seeking a Speech-Language Pathologist to evaluate and treat residents, document care, and collaborate with families and physicians. The role includes active clinical supervision of CFY staff and ongoing staff training within the facility.

The SLP will assess patients within 24 hours of referral, develop treatment plans, and ensure compliance with state practice acts and facility policies.

Qualifications

• Master's or doctoral degree in SLP or an equivalent credential.
• Active state licensure as an SLP or eligibility for licensure.
• Evidence of supervised clinical practicum and post-degree experience as required by state.

Responsibilities

• Evaluate patients within 24 hours of referral and document delays if any.
• Develop treatment plans and obtain physician approval for services.
• Treat patients per the physician treatment plan and record progress.
• Supervise CFY/CFY-style staff in direct patient care and training.
• Communicate patient progress with supervisors and health team members.
• Document evaluations, treatment notes, and discharge planning records.
• Instruct families and nursing staff in maintenance and discharge preparation.
• Comply with applicable state practice acts and facility policies.

Education

Master's or doctoral degree in SLP

Licensed as an SLP by the state

350 clock hours of supervised practicum (alternate licensure path)

9 months of supervised SLP services after degree

National examination in SLP (HHS approved)
