Job summary

Creative Solutions in Healthcare - West is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents through person-centered care.

You will evaluate swallowing and cognitive function, develop and implement treatment plans, provide AAC support, educate families and caregivers, and collaborate with other professionals to ensure quality care in a long-term care setting.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory agencies/licensing boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Familiarity with Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents in improving cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop individualized treatment plans.
• Implement AAC systems for severe expressive/language disorders.
• Provide training to clients, families, caregivers and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Demonstrate competency in speech therapy standards of practice.
• Communicate changes in resident's condition.
• Maintain accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other staff.
• Ensure no resident abuse or neglect and maintain confidentiality.
