Job summary

RHF (Retirement Housing Foundation) seeks a Service Coordinator for West Valley Towers in Van Nuys, CA. This full-time role identifies resident needs, coordinates social and health services, and develops health and wellness programs to enhance independence.

You will advocate for residents, build partnerships with community providers, and maintain records. Strong communication, advocacy, and organizational skills are essential for success in this role.

Qualifications

• Preferred: Bachelor's degree in Gerontology, Human Services, Social Work, Psychology or related field.
• Experience with seniors and diverse groups preferred.
• Strong communication and advocacy skills required.

Responsibilities

• Resident Assessment: Evaluate residents' needs to identify services that can enhance their quality of life and promote independence.
• Service Coordination: Establish and maintain connections with local service providers for access to social, health, and wellness resources.
• Program Development: Create and sustain educational health and wellness programs tailored to residents.
• Resource Management: Develop a directory of community resources and services for residents.
• Advocacy: Act as a resident advocate and liaise with community services.
• Community Liaison: Build relationships with community agencies and service providers.
• Communication: Facilitate open communication between residents and providers.
• Collaboration: Coordinate with management, other departments, and external agencies.
• Crisis Intervention: Provide information, referrals, and support in crisis situations.
• Referral Monitoring: Track referrals and ensure follow-up and delivery.
• Record Keeping: Maintain accurate records and reports.
