Responsibilities

• Assess residents to identify needs and services to enhance quality of life.
• Coordinate with local providers to access social, health, and wellness resources.
• Develop and sustain health and wellness programs for residents.
• Maintain directory of community resources and services.
• Advocate for residents and liaise with external agencies.
