RHF is seeking a Social Service Coordinator in Vacaville, CA to support residents’ well‑being by identifying needs and coordinating essential services. The role includes building relationships with community providers, developing health and wellness programs, and advocating for residents.

The position is full-time, 40 hours per week, non-exempt, with a base pay around $25 per hour and benefits. RHF emphasizes dignity, independence, and collaborative service delivery within its housing community.

Qualifications

• Bachelor’s degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field.
• Experience working with diverse groups of seniors and disabled individuals.

Responsibilities

• Assess resident needs to identify services that enhance quality of life and independence.
• Coordinate access to social, health, and wellness resources with community providers.
• Develop and sustain health and wellness programs for residents.
• Maintain a comprehensive directory of community resources and services.
• Advocate for residents and liaise with community services and management.
• Build relationships with community agencies and identify new services.
• Foster effective communication between residents and service providers.
• Collaborate with management and other departments to coordinate social services.
• Provide information, referrals, and support for crisis intervention.
• Track and follow up on resident referrals to external agencies.
• Maintain daily and monthly records documenting service coordination.
• Perform additional duties to support RHF’s mission.
