Job summary

RHF is seeking a part-time Social Service Coordinator to support residents of Valley View Apartments in Manitowoc, Wisconsin. The role focuses on assessing needs, coordinating services, and promoting health and wellness for aging residents.

The successful candidate will build relationships with community providers, develop educational programs, maintain resource directories, and advocate for residents while ensuring ethical and confidential practice.

Qualifications

• Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience with diverse groups of seniors and disabled individuals.
• Experience in social services is considered.

Responsibilities

• Assess residents' needs to identify services that can enhance quality of life and promote independence.
• Coordinate access to social, health, and wellness resources.
• Develop and sustain health and wellness programs for residents.
• Maintain a directory of community resources and services.
• Advocate for residents and liaise with service providers.
• Collaborate with housing staff and external agencies.
