Job summary

Element Care PACE in Lynn, MA is seeking an Information and Referral Coordinator to respond to inquiries about PACE services. You will gather details, educate callers, schedule enrollment steps and document outcomes in our CRM.

You'll build strong referral networks with health professionals, support enrollment across teams, and assist with data entry while traveling locally to designated senior housing buildings.

Qualifications

• High school degree or equivalent; associate degree preferred.
• 2-4 years of work experience; experience with seniors or caregivers preferred.
• Advanced computer skills including Microsoft Office and CRM software.
• Strong listening and interpersonal skills; able to build relationships.
• Excellent verbal and written communication skills.
• Excellent organizational and time management skills.
• Medical terminology experience preferred.
• Experience working with diverse backgrounds.

Responsibilities

• Respond to health and in-home service inquiries by phone, email, and mail.
• Schedule visits and screen referrals for enrollment; coordinate with enrollment coordinators.
• Document outcomes and actions in inquiries within CRM.
• Develop and maintain referral relationships with health professionals.
• Collaborate with marketing to standardize call responses and processes.
• Contribute to team meetings with enrollment strategy insights.
• Assist with data entry quality and special projects.
• Local travel to designated senior housing buildings.
• Perform other duties as required.
