Job summary

Neighborhood Healthcare PACE seeks a Referrals Specialist to coordinate all referrals and follow-up care for our PACE participants. You will receive, process, schedule, and track medical referral requests for in-house and outside services.

This full-time onsite role requires excellent communication, bilingual English/Spanish is highly preferred, and proficiency with CPT/ICD-10 and EMR systems. Pay ranges from $22.88 to $32.08 per hour, depending on experience.

Qualifications

• High school diploma/GED required.
• One year of clinical or healthcare experience required.
• Experience with referral authorization and data processing preferred.
• One year of experience working with elderly populations preferred.
• Bilingual English/Spanish highly preferred.
• Current BLS certification required upon hire; CPR and First Aid renewal annually.

Responsibilities

• Serves as the main point of contact for providers and clinic staff regarding referrals, authorizations, and scheduling.
• Prepares, processes, and completes referrals accurately and timely.
• Arranges transportation for participants to medical appointments.
• Communicates referral details and appointment information to participants and families.
• Tracks referrals in EMR and follows up on authorization requests.
• Reschedules missed appointments and notifies providers per policy.
• Acts as liaison between participants, clinics, specialists, hospitals and resources.
