Prisma Community Care seeks a Ryan White Case Manager to provide comprehensive case management for people living with HIV. You will assess needs, develop service plans, and coordinate medical and supportive services to help clients stay in care and achieve viral suppression.

The role emphasizes cultural competency, collaboration with internal and external programs, and documentation in EMR to ensure timely reporting and accountability.

Qualifications

• Bachelor’s degree in social work or related field required or four years of relevant experience with HS diploma.
• Ability to establish rapport with culturally diverse populations.
• Knowledge of HIV, housing, insurance, and barriers to care.

Responsibilities

• Conduct psychosocial and program eligibility assessments for Ryan White clients.
• Develop individualized service plans and monitor progress toward goals.
• Close gaps in medical care and support attendance at visits and treatments.
• Manage an active caseload with a 60% billable time target across clients.
• Document contacts and activities in EMR within required timeframes.
• Provide information and referrals to partner agencies for social/medical needs.
• Navigate insurance and patient assistance programs to expand access to care.
• Stay informed on HIV care trends and best practices; collaborate with teams.
• Work with department leadership on program-related tasks.
