Priority Health Care in the United States is seeking a compassionate Ryan White Non-Medical Case Manager to connect clients with essential supportive services and help them stay engaged in care.

You will conduct eligibility screenings, assess barriers to care, coordinate referrals to community resources, and advocate to remove obstacles to healthcare access. Collaborative work with community partners and thorough documentation per HIPAA is essential.

Qualifications

• Bachelor’s degree in any field or equivalent Experience.
• Three years HIV-related services with HS diploma/GED accepted as alternative.

Responsibilities

• Screen clients for initial and annual Ryan White program eligibility; maintain documentation and recertification records
• Assess social, economic, housing, food security, and transportation needs
• Develop individualized support plans; address barriers to health and care engagement
• Provide referrals to housing, food, transportation, and community services
• Follow up on referrals to ensure service access and outcomes
• Provide crisis intervention and advocacy with community agencies, landlords, social services, and government programs
• Provide information on HIV transmission prevention and disease management basics
• Support problem-solving around social determinants of health and medication adherence barriers
• Document assessments, support plans, and client interactions in the EHR per HIPAA and Ryan White standards
• Build relationships with community organizations and service providers; connect clients to resources
• Ability to work independently with minimum supervision

Ryan White Non-Medical Case Manager

Priority Health Care is seeking a compassionate Non-Medical Case Manager to join our Ryan White Program and connect clients with essential supportive services that enable them to stay healthy and engaged in care. You'll conduct eligibility screenings, assess barriers to care, coordinate referrals to community resources, and provide direct advocacy to remove obstacles to healthcare access. Your skills in problem-solving, resourcefulness, and knowledge of community systems are essential to supporting people living with HIV.

Responsibilities:

• Screen clients for initial and annual Ryan White program eligibility; maintain documentation and recertification records
• Conduct comprehensive assessments of social, economic, housing, food security, and transportation needs
• Develop individualized support plans; identify and address barriers to health and care engagement
• Provide referrals to housing, food, transportation, utility assistance, and community support services
• Follow up on referrals to ensure clients access services and achieve support plan outcomes
• Provide crisis intervention and advocacy with community agencies, landlords, social services, and government programs
• Provide information on HIV transmission prevention and disease management basics
• Support clients with problem-solving around social determinants of health and medication adherence barriers
• Document all assessments, support plans, and client interactions in the EHR system per HIPAA and Ryan White standards
• Build relationships with community organizations and service providers; connect clients to appropriate resources
• Ability to work independently with minimum supervision

Minimum Qualifications:

Applicants must meet one of the following qualification pathways:

• Education Pathway: A bachelor’s degree in any field;

OR

• Experience Pathway: A high school diploma or GED and three years of experience providing HIV-related services.

Additional Preferred Qualifications:

• Strong knowledge of community resources, social services programs, benefits enrollment, and housing resources
• Excellent interpersonal and communication skills with diverse, vulnerable populations
• Proficiency with Microsoft Office and ability to learn EHR and case management systems
• Strong organizational skills and ability to manage multiple cases and priorities
• Ability to build trust and provide culturally competent support
• Reliable transportation and flexibility for community outreach and home visits
• Bilingual ability in Spanish or Vietnamese is a plus
• Completion of HIV-specific training and cultural competency training within first year of hire
