Job summary

Priority Health Care is seeking a compassionate Ryan White Medical Case Manager to coordinate comprehensive healthcare services for clients living with HIV. You will screen eligibility, develop care plans, and connect clients to medical and support services while advocating across clinical and community settings.

You will document in the EHR per HIPAA and Ryan White standards, collaborate with providers, and build trust with diverse populations. Bilingual candidates are a plus.

Qualifications

• Licensed Social Worker with current licensure (RSW/CSW/LMSW/LCSW) and CSWE-accredited program.
• Licensed Nurse with current LA RN or LPN license.
• Bachelor’s degree in human services-related field with 1 year post-degree experience; Master’s preferred.
• Bachelor’s degree in liberal arts/general studies with 16 credits in human services and 1 year post-degree experience.

Responsibilities

• Screen clients for Ryan White eligibility; maintain documentation and recertification records.
• Assess medical, psychosocial, social, and financial needs.
• Develop individualized care plans with providers and partners.
• Refer to medical providers, mental health, substance use, housing, food, transportation services.
• Follow up on referrals to ensure access to services and adherence to care plans.
• Provide advocacy and crisis support in clinic, community, or home settings.
• Counsel on HIV management, medication adherence, and transmission prevention.
• Document assessments, plans, and interactions per HIPAA and Ryan White standards.

Ryan White Medical Case Manager

Priority Health Care is seeking a compassionate Medical Case Manager to join our Ryan White Program and coordinate comprehensive healthcare services for clients living with HIV.

You’ll conduct eligibility screenings, develop individualized care plans, coordinate referrals across medical and support services, and provide direct advocacy to help clients navigate the HIV care continuum. Your ability to connect clients with services, understand HIV disease and treatment, and build trust with diverse populations is essential to our mission.

Responsibilities:

• Screen clients for initial and annual Ryan White program eligibility; maintain documentation and recertification records
• Conduct comprehensive assessments of medical, psychosocial, social, and financial needs
• Develop individualized care plans in collaboration with providers and community partners
• Provide referrals to medical providers, specialists, mental health, substance use treatment, housing, food, and transportation services
• Follow up on referrals to ensure clients access services and achieve care plan goals
• Provide advocacy and crisis support in clinic, community, or home settings
• Counsel clients on HIV disease management, medication adherence, transmission prevention, and harm reduction
• Document all assessments, care plans, and client interactions in the EHR system per HIPAA and Ryan White standards
• Ability to work independently with minimum supervision
• Build relationships with community organizations; connect clients to HIV and support services

Minimum Qualifications:

Applicants must meet one of the following qualification pathways:

• Licensed Social Worker: Current licensure as an RSW, CSW, LMSW, or LCSW and graduation from a program accredited by the Council on Social Work Education;

OR

• Licensed Nurse: Current Louisiana licensure as a registered nurse (RN) or licensed practical nurse (LPN);

OR

• Human Services Degree and Experience: A bachelor’s degree in a qualifying human services-related field and one year of post-degree experience providing direct services to the HIV target population. A master’s degree is preferred;

OR

• Liberal Arts or General Studies Degree and Experience: A bachelor’s degree in liberal arts or general studies, including at least 16 credit hours in a qualifying human services-related field, and one year of post-degree experience providing direct services to the HIV target population.

Additional Preferred Qualifications:

• Knowledge of HIV disease, treatment options, medication adherence, and prevention strategies
• Knowledge of healthcare and community resources, insurance programs, and benefits
• Proficiency with EHR systems and case management software
• Excellent communication and problem-solving skills; ability to build trust with diverse populations
• Bilingual ability in Spanish or Vietnamese is a plus
• Completion of HIV-specific training within 6 months of hire
