Legacy Community Health Services in Houston is seeking a Lead RN Telephone Triage Nurse to act as the primary link between patients and care through careful telephone assessments and guidance. You will help patients understand options, receive appropriate medical attention, and navigate the healthcare system with confidence.

The role focuses on indirect patient care via telephone inquiries, following established policies and practice standards, using approved protocols, and scheduling timely

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Texas licensure as an RN.
• Current CPR certification required.

Responsibilities

• Accept difficult, urgent or emergency calls and triage using best practice guidelines.
• Document care and ensure accurate records per protocols.
• Schedule follow-ups based on patient condition.
• Provide nursing advice and patient education.
• Coordinate care with providers and clinical team.
• Maintain average call time of 8 minutes or less.
• Safeguard patient privacy and confidentiality.
• Use approved protocols during telephone advice.
