Job summary

Centene is seeking a Registered Nurse to join our Senior LTSS Service Care Manager team, coordinating care for complex populations in a remote role across the U.S. Strong care planning and collaboration with families, providers, and leadership are essential.

You will develop long-term care plans, monitor outcomes, educate families about options, and advocate for members to ensure high-quality care.

Qualifications

• Graduate from Accredited School of Nursing; Bachelor's degree preferred.
• 4-6 years of related experience required.
• RN licensure and/or NP licensure required; RUG certification within 90 days of hire.

Responsibilities

• Evaluate service needs of high-risk members and plan outcomes.
• Develop and assess long-term care plans with the care management team.
• Coordinate and manage interactions between members, families, and providers to ensure adequate care.
• Monitor and revise care plans to meet goals.
• Review member data to identify trends and improve care quality.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect, document, and maintain member information to ensure guidelines.
• Educate members and families on care options and benefits.
• Act as liaison and advocate between members, families, and healthcare providers.
• Engage with leadership to enhance quality of care.
