Centene is seeking a licensed nurse to perform in-home care management assessments for complex/high-acuity populations. You will develop personalized care plans, educate members and families, and coordinate with a care team to ensure high-quality, cost-effective services.

Responsibilities include monitoring plans, updating based on changes, and advocating for members while navigating state and federal guidelines. The role is hybrid with in-home visits and ongoing quality improvement involvement.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree in a related field.
• RN - State Licensure and/or Compact State Licensure required or NP - Nurse Practitioner licensure required.
• RUG certification must be obtained within 90 days of hire.

Responsabilités

• Evaluate service needs of high-acuity members and propose optimal plans.
• Develop and monitor long-term care plans in collaboration with the care team.
• Coordinate between member/family and care providers to ensure person-centered care.
• Monitor progress and update care plans to meet member goals.
• Collaborate with healthcare providers to adjust services as needed.
• Maintain all member information in compliance with regulations.
