Job summary

Centene is seeking a Senior LTSS Service Care Manager (RN) to assess and coordinate medical and support services for complex populations. The role is remote with NY alignment and a full-time schedule, offering competitive pay and benefits.

You will evaluate needs, develop long-term care plans, coordinate with families and providers, monitor progress, and ensure compliance while educating members on healthcare options.

Qualifications

• RN license in good standing and/or State Licensure applicable.
• 4–6 years of related LTSS/care management experience.
• Education from an accredited nursing program; Bachelor's preferred.

Responsibilities

• Evaluate needs of high-risk members and plan outcomes.
• Develop and assess long-term care plans with care team.
• Coordinate communication between members, families, and providers.
• Monitor and revise care plans to meet goals.
• Review data to identify trends and improve care quality.
• Collaborate with healthcare providers to facilitate services.
• Document and maintain member information for compliance.
• Educate members and families on healthcare options.
• Act as liaison and advocate between members and providers.
• Work with leadership to measure readiness with teams.
