Job summary

Centene is seeking a Senior LTSS Service Care Manager RN to assess and coordinate medical and supporting services for complex/high acuity populations in a remote US setting. The role focuses on long-term care outcomes and efficient care planning.

You will lead care planning, collaborate with providers, educate members and families, and ensure documentation and compliance while delivering care with a patient-centered approach.

Qualifications

• RN licensure in state or compact state licensing required, or NP licensure.
• RUG certification within 90 days of hire.

Responsibilities

• Evaluate the service needs of complex or high-risk members and recommend a plan for the best outcome.
• Develop and continuously assess ongoing long-term care plans/service plans.
• Collaborate with the care management team to identify providers and community resources.
• Coordinate and manage care between members, families, and care providers.
• Monitor care plans and member status, revising as necessary.
• Review member data to identify trends and improve quality care.
• Develop appropriate care plans based on referrals and intake assessments.
• Collaborate with healthcare providers to facilitate member services.
• Document and maintain member information to ensure compliance with guidelines.
• Educate long-term care members and their families on care options and benefits.

About This Role

The Senior LTSS Service Care Manager RN performs care management duties to assess and coordinate medical and supporting services for complex/high acuity populations. This role is ideal for experienced RNs looking to make a meaningful impact in long-term care.

Highlights

• Pay: $36 - $65/hour
• Location: Remote, US
• Schedule: Full Time, Day shift

What You'll Do

Requirements

• Graduate from an Accredited School of Nursing or Bachelor's degree preferred.
• 4–6 years of related experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required, OR
• NP - Nurse Practitioner - Current State's Nurse Licensure required.
• Resource Utilization Group (RUG) certification must be obtained within 90 days of hire required.
