Centene is seeking a Senior LTSS Service Care Manager (RN) to assess and coordinate medical and support services for complex populations. The role is remote with NY alignment and a full-time schedule, offering competitive pay and benefits.

You will evaluate needs, develop long-term care plans, coordinate with families and providers, monitor progress, and ensure compliance while educating members on healthcare options.

Qualifications

• RN license in good standing and/or State Licensure applicable.
• 4–6 years of related LTSS/care management experience.
• Education from an accredited nursing program; Bachelor's preferred.

About This Role

The Senior LTSS Service Care Manager (RN) performs care management duties to assess and coordinate medical and supporting services for complex populations. This role is ideal for experienced RNs looking to make a significant impact on patient care.

Highlights

• Pay: $36 - $65/hour
• Location: Remote, NY
• Schedule: Full Time

What You'll Do

• Evaluate the service needs of complex or high-risk members and recommend a plan for the best outcome.
• Develop and continuously assess ongoing long-term care plans and collaborate with the care management team.
• Coordinate and manage communication between members, families, and care providers.
• Monitor care plans and member status, revising as necessary to meet goals.
• Review member data to identify trends and improve quality care.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect, document, and maintain member information to ensure compliance with guidelines.
• Provide education to long-term care members and their families on healthcare options.
• Act as a liaison and advocate between members, families, and healthcare providers.
• Engage in a collaborative process with leadership and cross-functional teams to measure readiness.

Requirements

Education:

• Graduate from an Accredited School of Nursing or a Bachelor's degree preferred.

Experience:

• 4–6 years of related experience required.

Licenses & Certifications:

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required, OR
• NP - Nurse Practitioner - Current State's Nurse Licensure required.
