Job summary

Superior HealthPlan is seeking an experienced RN Care Manager to assess and coordinate medical and support services for complex adult members. You will develop personalized care plans, educate families, and liaise with providers to ensure high-quality, cost-effective care.

The role involves home visits, remote coordination from a home office, and a Monday–Friday schedule. Prior experience in care management and a valid RN license are required.

Qualifications

• RN with 4–6 years of clinical nursing experience (BSN preferred)
• Strong documentation and communication skills
• Ability to coordinate complex care across providers and services

Responsibilities

• Evaluate needs of high-risk members and propose optimal care plans
• Develop and monitor long-term care/service plans with care teams
• Coordinate between member/family and providers for person-centered care
• Monitor progress and update care plans as needs change
• Identify trends in member data to improve quality and efficiency
• Collaborate with healthcare providers to adjust services as needed
