Job summary

Superior HealthPlan is seeking an experienced Senior Care Manager RN to assess, plan, and implement complex care management for members, developing personalized care/service plans and educating families on available benefits.

The role focuses on chronic disease management and coordination with providers to deliver high-quality, cost-effective care in a Monday–Friday 8:00 a.m.–5:00 p.m. CST schedule, with potential Medicaid/STAR CHIP experience preferred.

Qualifications

• Requires a Nursing degree and 4-6 years of related RN experience.
• Experience with care management and member education.
• Strong clinical documentation and communication skills.
• Ability to work in a fast-paced healthcare environment.

Responsibilities

• Develops and continuously assesses care plans with providers to address unmet needs.
• Coordinates between member/family and care team to ensure person-centered care.
• Monitors progress and revises care plans to meet goals.
• Performs outreach to assess needs via telephonic, digital, or home visits.
• Educates members and families on disease processes and benefits.
• Collaborates with leadership to improve care delivery in a cost-effective manner.
• May precept new hires and guide onboarding.
• Ensures compliance with policies and standards.
