Job summary

Superior HealthPlan is seeking an experienced RN Care Manager to assess and coordinate care for complex/high acuity Duals AIP adults in the South Dallas area. You will develop personalized care plans, perform home health visits, and educate members and families on services and benefits.

The role is hybrid, with schedule Mon–Fri 8–5 CST and mileage reimbursement. Requires BSN preferred, RN license, and 4–6 years of clinical experience.

Qualifications

• RN with 4–6 years of related clinical nursing experience.
• BSN preferred.
• Current State Licensure (RN) or equivalent.
• RUG certification must be obtained within 90 days of hire.

Responsibilities

• Evaluate and coordinate care plans for complex/high-risk members.
• Develop and assess long-term care/service plans and coordinate with providers.
• Coordinate between member/family and care team to ensure appropriate, person-centered care.
• Monitor progress and revise care plans as needed to meet goals.
• Educate members and caregivers on services, benefits, and community resources.
