Job summary

Centene Corporation is seeking an RN Care Manager to assess and coordinate care for dual-eligible adults with complex medical needs in the Dallas area. You will conduct home visits, develop personalized care plans, and collaborate with the care team to ensure high-quality, cost-effective services.

The role offers a hybrid schedule, no on‑call duties evenings or weekends, and reimbursement for mileage within the service area. Requires RN license and 4–6 years of related experience; BSN preferred.

Qualifications

• Graduate from an accredited school of nursing.
• 4–6 years of related clinical nursing experience.
• BSN preferred and active RN license is required.

Responsibilities

• Evaluates the most complex/high risk members to determine best outcomes.
• Develops and updates long‑term care/service plans with providers.
• Coordinates between member/family and care team to ensure appropriate care.
• Monitors progress and revises care plans to meet goals.
• Reviews referrals and intake assessments to inform care plans.
• Educates members and families on diseases, services, and benefits.
• Acts as liaison between members, physicians, and facilities.
• Coordinates community resources and service authorizations.
