Job summary

Johns Hopkins Intrastaff seeks an experienced Case Manager (RN/ACM/CCM preferred) to coordinate patient-centered care across the continuum. You will collaborate with the interdisciplinary team to manage transitions from pre-admission through post-discharge, identifying barriers and optimizing clinical resources for better outcomes.

Required: RN with a Maryland nursing license; 5–7 years nursing and 1+ year in case management or discharge planning.

Qualifications

• Completion of an accredited BSN Nursing Program (RN).
• Master of Science in Nursing (MSN) preferred.

Responsibilities

• Collaborates with patients/families, social workers, physicians and the interdisciplinary team to coordinate patient-centered care.
• Leads transition planning for patients within an assigned caseload from pre-admission through post-discharge.
• Identify and eliminate barriers to delivery of clinical services with patient, family and care team.
• Coordinate and lead multidisciplinary rounds and communicate handoffs to ensure continuity of care.
• Monitor clinical resources and transition planning to improve quality and reduce readmissions.
