Job summary

Johns Hopkins Medicine in Baltimore seeks a Home Care Coordinator Discharge Planner to coordinate services for patients transitioning from hospital to home. You will act as a discharge planning resource without direct clinical care, supporting families and care teams.

40 hours per week, days, with Johns Hopkins Health System commitments. A BSN and active RN licensure are required, along with CPR certification and home care/discharge planning experience. Eligible for comprehensive benefits.

Qualifications

• BSN required and RN licensure active.
• CPR certification is required.
• Two years of clinical nursing experience, home care or discharge planning preferred.
• Knowledge of Medicare Conditions of Participation and home care regulations preferred.

Responsibilities

• Coordinate home care services and resources for patients transitioning from hospital to home.
• Serve as a discharge planning resource without providing hands-on clinical care.
