Johns Hopkins Medicine is seeking a Case Manager (RN) to coordinate patient-centered care across the continuum. You will collaborate with patients, families, social workers and physicians to facilitate safe transitions from pre-admission through post-discharge.

In this role you will lead transition planning, work to remove barriers to services, and advocate for resources. A BSN and current RN license are required, with 2+ years nursing and 1 year case management experience preferred.

Qualifications

• Completion of an accredited BSN Nursing Program (RN) is required.
• A minimum of 2 years clinical nursing experience and 1 year case management experience.
• Valid state RN license required.
• CPR certification required.
• Certification in case management (ACM‑RN or CCM) preferred; must obtain within 1 year.

Responsabilités

• Coordinate patient-centered care across the continuum with interdisciplinary team.
