Johns Hopkins Medicine in Annapolis, MD, is seeking a Home Care Coordinator Discharge Planner to coordinate post-acute home care for patients transitioning from hospital to home. This role acts strictly as a discharge planning resource, without hands-on clinical care, and collaborates with hospital teams to arrange needed resources.

The ideal candidate holds a BSN with RN licensure, CPR certification, and at least two years of clinical nursing, preferably with home care or discharge planning

Competenze

• Bachelor of Science in Nursing (BSN) required.
• Licensure in clinical discipline (RN).
• Active CPR.
• Two years experience in clinical nursing required, previous home care or home care discharge planning experience preferred.
• Knowledge of Medicare Conditions of Participation preferred.
• Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.

Mansioni

• Coordinate home care services and resources for patients transitioning from hospital to home.
• Act as a discharge planning resource without providing hands-on clinical care.
• Collaborate with hospital staff to arrange patient resources.
• Support patients and families in care planning and coordination.
