Qualifications

• Completion of an accredited BSN Nursing Program (RN), required.
• Minimum 2 years of experience in clinical nursing required and minimum of 1 year of experience in case management required.
• Valid state license as a registered professional nurse required.
• CPR- Basic CPR required
• Certification in case management by a national accreditation organization i.e. – Registered Nurse (ACM – RN) / Certified Case Manager (CCM), preferred. Required to obtain within 1 year of hire.

Responsibilities

• Collaborate with patients/families, social workers, physicians and interdisciplinary team to coordinate patient-centered care across the continuum.
• Lead transition planning and coordinate resources to facilitate safe and timely post-discharge progression.
