Job summary

Centene Corp. is seeking a Hybrid RN Care Manager to assess and coordinate care for complex/high acuity members in Hidalgo County, TX. You will develop personalized care plans and educate members and families on available services and benefits.

This role includes home visits, coordination of LTSS needs, and flexible scheduling from a home office. Requires 4–6 years of nursing experience; BSN preferred and strong documentation skills.

Qualifications

• Graduate from an accredited school of nursing with active RN license.
• 4–6 years of related clinical nursing experience; BSN preferred.
• Strong clinical documentation and communication skills.
• Ability to work a hybrid schedule and coordinate care.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan.
• Develops and assesses ongoing long-term care plans with providers and community resources.
• Coordinates between member/family and care team to ensure appropriate person-centered care.
• Monitors care plans and condition changes; revises plans as needed to meet goals.
• Acts as liaison between member, family, physician, and facilities/agencies.
• May perform home visits and educate members and families on services and benefits.
