Job summary

Centene Corporation is seeking a nurse care manager to assess and coordinate medical and support services for complex/high acuity populations in a hybrid role across the Harlingen, Mercedes, Weslaco area. The role focuses on developing personalized care plans and educating members and families on available services.

Requires RN licensure (or NP) and a BSN with 4–6 years of related experience in care management.

Qualifications

• Requires RN licensure or NP licensure with state credentials.
• Bachelor's degree in Nursing; BSN preferred.
• 4–6 years of related care-management experience in long-term care settings.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses long-term care plans with care management team to identify providers and resources.
• Coordinates between member/family and care provider team to ensure person-centered care.
• Monitors progress toward goals and revises care plans as needed with stakeholders.
• Educates members and families on disease processes, services, and benefits; acts as liaison with providers.
