Qualifications

• BSN required; RN license in Virginia.
• Minimum five years of clinical nursing experience.
• Three years of case management experience.
• Case Management Certification required.
• Active RN Licensure for the state of Virginia.
• CPR certification within 6 months.

Responsibilities

• Coordinate discharge planning and transitions of care.
• Collaborate with physicians and multidisciplinary team.
• Monitor length of stay and resource utilization; identify improvements.
• Mentor new hires and serve as preceptor for others.
• Address social determinants of health (SDOH) and connect to community resources.
