Job summary

Working with physicians, nurses and the care management team, you will develop tailored discharge plans, address barriers, and monitor resource use to optimize outcomes and cost efficiency.

Qualifications

• MSW degree and LMSW licensure; CCM/ACMA certification preferred.
• Minimum three years of clinical social work with caseload management experience.
• Experience with CMS regulations, payer guidelines, and accreditation standards.
• Proficiency in electronic health records (EHR).

Responsibilities

• Create and manage discharge plans addressing psychosocial, functional, safety needs.
• Collaborate with physicians, nurses, and the care team for patient management.
• Comanage caseloads with RN Case Managers and coordinate post-acute care.
• Identify social determinants of health needs and refer to community resources.
• Monitor length of stay and optimize resource utilization.
• Communicate denials/approvals and document in data systems.
