Job summary

UKG is seeking a Registered Nurse Senior Case Manager to coordinate patient care across the continuum in Virginia. This role combines clinical expertise with post-acute care planning to ensure safe, timely transitions.

You will lead discharge planning, collaborate with physicians and the care team, and manage a caseload while addressing social determinants of health to optimize outcomes. Qualifications include a BSN (Master's preferred), Virginia RN license, 5+ years of nursing and case

Qualifications

BSN required; master's degree in a related field preferred.
Active RN licensure in Virginia; CPR within 6 months.
Minimum five years of clinical nursing and case management experience.

Responsibilities

Coordinate patient care across the continuum and transitions of care.
Develop and implement discharge plans for assigned caseload.
Collaborate with physicians, nurses, and social workers; participate in performance improvement.
Identify and address social determinants of health (SDOH) and refer to community resources.
Lead discharge process and communicate effectively with care team and families.
Ensure CMSA standards and hospital utilization policies are followed.
