Job summary

Renown Health is seeking a Case Manager who works with an interdisciplinary team to assess, plan, implement, and evaluate post-acute care options to meet patient needs and support timely discharge. This role focuses on utilization management, including UR assessment within 24 hours of admission, discharge planning, and coordination across the care continuum.

Qualifications

• Must be proficient in English (reading, writing, speaking).
• Nevada RN license required; BSN preferred.
• One year RN experience; case management or UR/QA preferred.
• CCM or ACM certification preferred.
• Proficiency with Microsoft Office Suite (Outlook, PowerPoint, Teams, Excel, Word).

Responsibilities

• Lead case management activities across pre-acute to post-acute care.
• Perform UR assessments within 24 hours of admission.
• Coordinate discharge planning to ensure timely, appropriate transitions.
• Document care plans and communicate with the interdisciplinary team.
• Identify barriers to discharge and facilitate options with payers.
