Job summary

Chesapeake Regional Healthcare is seeking a Care Manager to coordinate patient care across the continuum during assigned shifts. The role blends clinical expertise with post-acute care knowledge to support safe, timely, and cost-effective transitions.

The Care Manager will perform discharge planning, quality management, and resource utilization while collaborating with a multidisciplinary team to achieve optimal outcomes. This PRN role has no guaranteed hours.

Qualifications

• RN licensure required unless you hold a degree in Social Work
• Minimum one year of clinical experience in an acute care setting
• Acute care case management, discharge planning, or care coordination highly preferred
• Must be self-directed with strong organizational skills

Responsibilities

• Demonstrates clinical knowledge and skills to perform duties
• Interprets clinical data to prioritize patient needs
• Collaborates with patients, families, and care team
• Plans and evaluates care management strategies for resource utilization
• Develops individualized discharge plans for assigned patients
• Coordinates with physicians, nurses, social workers for discharge planning
• Identifies social determinants of health and connects to community resources
