National Church Residences, the nation’s largest provider of affordable senior housing and health care services, seeks Service Coordinators for Redeemer Community in Dayton, OH. This part-time role (25 hours/week) focuses on coordinating resident services, engaging with community providers, and delivering high-quality support to seniors.

Qualificações

• Education: Bachelor's degree in social work or related field preferred.
• Two+ years experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsabilidades

• Advocate, organize, problem-solve and deliver results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and seek out new services for residents; identify low-cost providers or discounts.
• Engage residents to identify needs and refer to community services.
• Develop a Resource Directory of state/local service providers.
• Sponsor educational events on health care, agency support, life skills and referrals.
• Provide quality customer service and address residents' concerns and needs.
• Assist residents in obtaining community services such as meals, transportation, and personal services.
• Monitor feedback from residents and their families; maintain records.
• Manage internal and external relationships with strong communication.
