Job summary

Health & Human Services Commission in Richmond, TX is seeking a QIDP III to oversee the ISP process for individuals with complex needs, centered on the person’s preferences, strengths, and needs. You will coordinate the interdisciplinary team, monitor progress, and ensure timely follow-up.

You will lead IDT meetings, document assessments, and ensure adherence to ICF/IID standards while maintaining high-quality documentation and safeguarding the rights of individuals.

Qualifications

• Bachelor’s degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field.
• At least one (1) year of experience working directly with persons with intellectual disabilities or other developmental disabilities.

Responsibilities

• Oversee the ISP process for all individuals on the assigned caseload, including ISP preparation, addendum and annual ISP meetings.
• Lead and coordinate IDT meetings and ensure timely follow-up for recommendations.
• Monitor assessments to determine functioning, preferences, strengths and needs.
• Coordinate with the individual and LAR with staff to develop and deliver needed supports and services.
• Ensure follow-up for recommended supports, services, or programs to meet individuals’ needs.
• Observe interactions across settings and collect data on ISP implementation and effectiveness.
• Train and coach staff on service delivery and behavior management; provide leadership and mentorship.
