PACS seeks a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate with physicians, nursing staff, and families. The role includes supervising PT assistants, delivering in-services, and documenting care in medical records.

Education requirements include a Bachelor’s in Physical Therapy with possible Masters/Doctorate; proficiency with Net Health and Point Click Care; commitment to infection control and safety.

Qualifications

• Bachelor’s degree in Physical Therapy (Masters/ Doctorate preferred).
• Proficient with Net Health and Point Click Care systems.
• Ability to read and interpret policy and procedure manuals.
• Strong communication with managers, coworkers and families.
• Solid math skills for documentation and patient care.
• Ability to solve practical problems and interpret instructions.

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise Physical Therapy Assistants in resident care and related activities.
• Communicate progress and discharge plans with physicians, nursing staff, residents, and families.
• Complete timely, accurate documentation meeting facility and payer requirements.
• Educate residents and families on home exercise programs and safety techniques.
• Provide in-services and training to staff as needed.
