PACS is seeking a Staff Physical Therapist to evaluate residents, develop and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.

You will supervise PT assistants, train staff, and coordinate with physicians, nursing staff, and families to plan discharge and ensure quality care. The role emphasizes safety, documentation, and program development within a collaborative team.

Qualifikationen

• Bachelor’s degree in Physical Therapy; masters/doctorate preferred.
• Licensed as a Physical Therapist in the state; continuing education required.
• Proficiency with Net Health and Point Click Care software.

Aufgaben

• Evaluate residents to determine PT needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and train staff on therapy procedures.
• Communicate progress, goals, and discharge plans with physicians, staff, residents, and families.
• Complete accurate documentation in accordance with policies and payer requirements.
