Job summary

PACS Group is seeking a Staff Physical Therapist to evaluate and treat residents, collaborate with families and the health team, and maintain medical records documentation. You will lead in-services for facility staff and supervise PT assistants, ensuring high-quality care and safety.

The role requires a PT license, strong documentation skills, and experience coordinating with physicians and other team members. Residency facility setting and patient-centered approach are emphasized.

Qualifications

• Bachelor’s degree in Physical Therapy; Master’s or Doctorate preferred.
• Licensed Physical Therapist as required by state.
• Proficiency with Net Health and Point Click Care software.

Responsibilities

• Assess residents to determine PT needs and rehabilitation potential.
• Develop and update individual treatment plans to improve mobility and independence.
• Supervise PT assistants and train staff as needed.
• Communicate progress with physicians, nursing staff, residents, and families.
• Maintain compliant documentation and infection control standards.
