Job summary

PACS is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and collaborate with physicians, families, and the health care team. The role requires supervision of PT assistants and maintaining accurate documentation.

You will educate residents and families on home programs, support quality improvement, and participate in care conferences to optimize rehabilitation outcomes.

Qualifications

• Licensed Physical Therapist in the state.
• Bachelor's degree in Physical Therapy; Master/Doctorate preferred.
• Proficient with Net Health and PointClickCare systems.
• Ability to read and interpret procedures and policies.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate progress, goals, and discharge plans with families and physicians.
• Maintain compliant documentation per regulations and payer requirements.
• Educate residents and families on home exercise programs and safety.
• Assist with staff training and quality improvement initiatives.
