PACS is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, supervise PT assistants, and coordinate with physicians, families, and the care team to support mobility and independence.

The role requires a state license, a PT degree (Bachelor’s required; masters/doctorate preferred), and proficiency with PT documentation systems such as Net Health and PointClickCare. Excellent communication and safety practices are essential.

Qualifikationen

• Bachelor’s degree in Physical Therapy; advanced degrees preferred.
• Licensed in state; maintain licensure and CE requirements.
• Proficient with Net Health and Point Click Care.
• CPR certification, if required by licensure.
• Ability to read technical procedures and policy manuals.

Aufgaben

• Evaluate residents to determine PT needs and rehab potential.
• Develop, implement, and update treatment plans to improve mobility and independence.
• Supervise PT assistants and aide staff.
• Communicate with physicians, nursing staff, and families about progress and discharge planning.
• Complete timely, accurate documentation per policies and payer requirements.
• Maintain a safe treatment environment and infection control practices.
• Educate residents and caregivers on home programs and safety.
