Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, communicate with families and physicians, and maintain documentation in medical records. The role includes training facility staff and supervising PT assistants to ensure high-quality resident care.

The position requires a state license, a degree in Physical Therapy (masters or doctorate preferred), and proficiency with Net Health and Point Click Care.

Qualifications

• Bachelor’s degree in Physical Therapy; advanced degrees preferred.
• Licensed as a Physical Therapist in the state.
• CPR certification, if required by licensure.
• Proficiency with Net Health and Point Click Care systems.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and aides in resident care.
• Communicate with physicians, nursing staff, families; discharge planning.
• Maintain timely, compliant documentation.
• Educate residents and families on home exercise and safety.
• Maintain safe treatment environment and infection control.
• Contribute to quality assurance and program development.
