Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat patients, communicate with families and physicians, and maintain comprehensive documentation in medical records. The role includes in-services and training for facility staff on an ongoing basis.

Responsibilities include timely evaluations, treatment planning, discharge planning, and supervising PT assistants, with emphasis on compliant, evidence-based care and collaboration with the health team across the facility.

Qualifications

• Bachelor’s degree in Physical Therapy; state license required.
• Casamba and Point Click Care experience or proficiency.
• Active licensure and ongoing CE requirements in state.
• Ability to communicate with managers, families and staff.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per the approved plan and document progress.
• Supervise PT assistants and student interns in direct care.
• Coordinate discharge planning and caregiver training for home programs.
• Record evaluations, daily notes, and progress in medical records.
• Communicate patient progress with supervisor and health team.
• Provide staff in-services and training as needed.
