Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat patients in a post-acute setting. You will communicate with families, physicians, and health team members, and document services in medical records. The role includes training facility staff and supervising PT assistants as needed.

Responsibilities include discharge planning, equipment recommendations, and collaboration with the care team to ensure quality outcomes. Must maintain licensure and comply with state practice acts.

Qualifications

• Bachelor’s degree in Physical Therapy required. Licensure to practice in the state.
• Masters or Doctorate in Physical Therapy preferred.
• Proficient computer skills including Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans from referring physician.
