Qualifications

• Licensed Physical Therapist in the state.
• Continuing education requirements must be met as per state practice.
• Maintain license in good standing with state board.

Responsibilities

• Evaluate patients within 24 hours of referral and develop treatment plans.
• Supervise PT assistants, aides and students.
• Record evaluations, daily/weekly notes and recertification docs per policy and Act.
• Instruct families and staff on maintenance and caregiver training for discharge.
• Participate in discharge planning and care conferences.
