Job summary

Qualifications

• High school graduate (Associate or higher preferred).
• 2+ Years experience in a Home Care or Service Coordination role.
• Demonstrated capability maintaining strict confidentiality.
• Proven ability to generate leads and monitor referrals.
• Strong typing and computer skills.
• Comfortable with closing/asking for business.
• Well organized, accurate, and attentive to detail.
• Excellent communication, public relations and follow up skills.
• Experience with HHA Exchange and EVV.
• Must Speak Fluent English.

Responsibilities

• Provide coordinated care to patients by developing, monitoring, and evaluating home health care plans.
• Ensure high level of care by home health aides.
• Schedule agency aides and ensure shifts are covered.
• Manage last-minute calls and coverage.
• Develop on-call pool of aides with HR.
• Communicate with referral sources and case managers for excellent service.
• Match client needs with appropriate caregiver(s).
• Manage caseload of up to 100 patients.
• Review care plans with patients and caregivers.
• Resolve grievances and complaints from caregivers and clients.
• Address over-utilization of hours.
• Report personnel performance issues.
• Ensure caseload retention.
• Contribute to team efforts by achieving related results.
