Job summary

Elite Home Health Care, Inc. is seeking a Coordinator to ensure smooth coordination of home care services for patients in the New York area.

You will manage an assigned caseload, coordinate service placement between clients and caregivers using HHA eXchange, and support timely, high-quality care delivery. You'll work with team members to address staffing issues, update records, and participate in on-call duties.

Qualifications

• General experience in healthcare preferred.
• Proficient with Microsoft Office, HHA Exchange, and internet.
• Strong leadership, organization, and communication skills.
• Ability to multitask in a fast-paced environment.
• Flexible schedule to meet department and patient needs.

Responsibilities

• Manage assigned caseload and ensure high-quality home care.
• Coordinate placement of services between clients and caregivers using HHA eXchange.
• Maintain caseload management for timely care delivery.
• Attend departmental trainings and stay updated on policies.
• Assist team lead with coverage and staffing issues.
• Discuss scheduling, availability, and attendance with staff.
• Update patient and caregiver records in HHA eXchange.
• Verify caregivers' attendance; prioritize calls based on patient needs.
• Handle inquiries from contractors and clients; discuss patient status with nurse care managers.
• Receive caregiver complaints and collaborate on resolution.
• Assist with on-call duties as scheduled.
